Group Member Evaluation Form
MIST 5620: Business Intelligence
Name: _____________________ Course: _________. Semester / Year: ______ Group Name: _________________
Please indicate the effort/contribution of every individual working in your group on the group work for this course. Use a 0-100 point scale. Every individual can have any score from 0 to 100. It is important to evaluate whether you feel that each individual (INCLUDING YOURSELF) attended and contributed to the group meetings, was involved in the thought processes of the group, contributed to each group assignment (including the projects), and, in general was an active member of the group. Criteria can be based on the amount of time and effort expended, attendance at meetings, willingness to take on responsibility, cooperative attitude, attitude in general, etc. If a group member did not contribute, rate him/her zero. While the criteria may seem fuzzy, and though this is a subjective process, I do use this information in determining an individual's credit toward the group portion of his/her course grade. I use the ratings to develop an individual's group coefficient on a scale from 0-1 that I multiply the group points by for each individual's score.
If anyone receives a score that is 10 points or more less than the average score, explain below.
Make sure you rate everyone in your group, including yourself! Thank you.
